Subject to the provisions of Article (22) of this Law, the application for initiation of proceedings shall be accompanied by the following attachments: 1. Data, information and documents indicating that the necessary conditions have been met to submit the application for initiation of proceedings. 2. A brief explanation of the plan, indicating its conditions, the implementation method, the guarantees for its implementation, if any, and the schedule for implementation. 3. A summary of the contracts and agreements proposed to be signed between the debtor and creditors to implement the plan. 4. In the event that the Creditors' Committee is formed according to the ranking of creditors, a list of the members of such committee shall be attached, indicating the name of the representative for each group of creditors, their electronic and regular e-mail and phone numbers and documents indicating the appointment of these members and representatives and the limits of their authorization from creditors. 5. Procedures for calling a meeting of creditors to discuss the plan, the voting mechanism and persons who have the right to vote. 6. In the event that some creditors initially approve the plan, a list of the names of these creditors shall be provided according to their ranking, the amount of the debtor's debts with respect thereto, their guarantees and their percentage of the debtor's total debts. The applications submitted by the regulatory authority shall be exempted from submitting the data, information and documents referred to in Clauses (1) (4) of this Article.
